Retatrutide, a Triple Hormone Receptor Agonist, for Treatment of Obesity

New England Journal of Medicine, 2026 · paper · Clinical trials

A phase 3 randomized trial of once-weekly retatrutide versus placebo for 80 weeks in 2339 adults with obesity. Weight fell 23.7–25.0% at the higher doses versus 3.9% with placebo, with less knee pain and fewer sleep apnea events.

Substance The abstract prints the 12-mg hybrid-estimand change in apnea–hypopnea events as positive: "were −22.9, −34.3, and 31.7 versus −9.9 (differences, −24.4 and −21.9)"; Table 2 and the Results text give −31.7.

Judge's note: The page 1 image shows "and 31.7" with no minus sign, while the neighbouring values carry one, so this is not an extraction artefact. Table 2 (page 8) prints −31.7 (−38.2 to −25.2) and the page 6 text prints −31.7. The stated difference of −21.9 only fits a negative value.

Substance "From July 2023 through April 2026, a total of 2339 participants underwent randomization" does not fit a trial with "80 weeks of treatment, and 4 weeks of follow-up" that was "published on September 29, 2026".

Only the authors can say what is right here.

Judge's note: Read on the page 1, 2 and 4 images. April to the end of September 2026 is at most about 26 weeks, far short of the 80 weeks of treatment plus 4 weeks of follow-up, yet 84.7% are reported as completing the trial and week-104 extension data are given. Reference 33 is a news release dated December 11, 2025 that already reports phase 3 results. The end date of randomisation therefore appears to be wrong, or the date range describes trial conduct and not randomisation. Only the authors can supply the correct date.

Judge's note: The page 4 image shows "2.2%" with no minus, while the other three values in the sentence carry one. In Figure 1A on page 6 the placebo value in the EE column is labelled −2.2. As printed, the text reads as a weight gain.

Judge's note: The page 9 upright image shows "−15.7 (−6.7 to −14.8)" as printed, so this is not an extraction artefact. The neighbouring columns have a half-width of about 1 cm: −20.1 (−21.1 to −19.2), −21.6 (−22.6 to −20.5), −4.6 (−5.5 to −3.6). With −16.7 the midpoint is −15.75, matching the estimate; a leading "1" was dropped.

Judge's note: Read page 1, page 3 and the upright page 9 images. The abstract prints 1 to 10; the Methods and the Table 2 footnote both print 0 to 10 for the same pain subscale. Nothing in the article describes a different version or transformation for the abstract. Two of the three places agree, so the fix is evident.

Judge's note: Footnote † read on the page 11 image; it ends "Additional details are provided in Table S7." The text layer for page 7 cites deaths to "Table 3 and Table S9". Table S7 is cited on pages 5 and 6 for anthropometric targets, the BMI ≥35 subgroup and cardiometabolic measures under the treatment-regimen estimand. The two pointers for death details conflict, and the footnote's S7 is the likelier error. The supplement is not among the files, so which table actually holds the death details cannot be confirmed; hence an author query.

Judge's note: Read the page 6 image. The sentence is about the "mean change in the AHI events per hour" yet ends with "points". Table 2 labels the AHI difference "events/hr", and the parallel hybrid-estimand sentence gives no "points". "Points" is the WOMAC unit, carried over from the preceding sentence. Judge's note: Checked the page 10 image (Discussion text and the Figure 2B pies: 89.4%, 91.9%, 93.2%) and the Results on the page 6 image ("89.4 to 93.2%"). The sentence is framed as "across all retatrutide groups", but the 4-mg value is below 90%. A pooled retatrutide figure would be about 91.5%, so the authors may have meant a pooled value or the two higher doses; the wording does not say so. Slight overstatement in wording.

The Results give the range as 5.7 to 21.1%. The Discussion sentence has no dose qualifier and no "up to", so it reads as applying to trial participants generally; the observed proportion across the three retatrutide groups is about 15%. Page 9 image: the IWQOL-Lite-CT rows sit under the key-secondary heading with 4-mg values, and the table footnote says differences were significant for all primary and key secondary end points. Judge's note: Read on the page 4 and page 11 images. With the randomised denominator that footnote ‡ declares, 65/584 is 11.13%, which matches the text's 11.1. The table's 11.2 matches the safety denominator instead (65/582 is 11.17%). The other cells in the row fit the randomised denominators (4.1, 6.5, 4.6). Judge's note: Sentence read on the page 12 image. The Results on page 4 give the 4-mg change as −17.6% (treatment-regimen) and −19.0% (efficacy). Figure 1B gives BMI <30 for 4 mg as 40.1% under the treatment-regimen estimand. So the unlabelled 19% matches the efficacy estimand, while "more than 40%" matches the treatment-regimen value. It could also be an efficacy-estimand value, but that figure is in the supplement, which is not among the files. Elsewhere the Discussion labels each number with its estimand. This is an unlabelled, possibly mixed, estimand rather than a wrong number.

Wording "among the participants who switched to a maximum tolerated dose of 9 mg or 12 mg from placebo was −18.9%, −25.7%, −28.7%, and −29.9%, respectively" gives four values for what is worded as a single group, with no four-item list for "respectively" to refer to.

Judge's note: Page 5 image confirms the sentence. Table 2 (page 8) shows that −18.9 is the placebo-to-retatrutide group and that −25.7, −28.7 and −29.9 are the original 4-mg, 9-mg and 12-mg groups. The numbers agree with the table; the sentence misattributes them all to the group that switched from placebo. The wording needs the authors' revision.

Judge's note: Read the Discussion on the page 10 image and the Results and Table 2 in the text layer. The Results list waist circumference, systolic blood pressure, fasting insulin, hsCRP, triglycerides and non-HDL cholesterol. LDL does appear in Figure 2A (−13.2, −18.7, −16.3 vs −4.7), marked as not controlled for type I error, so the LDL part is supported. Diastolic blood pressure appears only in the Introduction, describing the phase 2 trial. The Results sentence also points to Figs.
