Hypothermia versus Normothermia after Out-of-Hospital Cardiac Arrest

2021 · paper · Clinical trials

A trial randomising 1,900 comatose adults after out-of-hospital cardiac arrest to cooling at 33°C or targeted normothermia. Death at 6 months was similar (50% vs 48%), and arrhythmia was more common with hypothermia.

1 finding

Substance Table 2 prints the normothermia arrhythmia rate as "152/921 (16)", but 152/921 = 16.50%, which rounds to 17, the figure given in the Abstract and Results ("24% vs. 17%").
  • p. 9 Table 2, Arrhythmia resulting in hemodynamic compromise, Normothermia column
    152/921 (16)
  • p. 8 Results, Adverse Events
    (in 24% vs. 17%; P<0.001)
  • p. 1 Abstract, Results
    (24% vs. 17%, P<0.001)
  • p. 8 Adverse Events text
    in 24% vs. 17%; P<0.001

Judge's note: I read the page 9 image of Table 2 and the page 1 and page 8 crops. 152/921 = 16.504%, which is above the midpoint, so this is not a rounding tie. The other table cells are rounded conventionally, not truncated (140/881 = 15.9 printed as 16; 87/881 = 9.9 printed as 10). The text and abstract say 17% for the same count, so the table cell is the inconsistent one.

6 minor (typos and the like; most checked by the judge only)
Wording Methods: "score of <4 on the Full Outline of Unresponsiveness [FOUR] scale,12 which ranges from 0 to 4". The 0–4 range is the motor component, which Table 1 footnote ‖ correctly calls "FOUR motor scores range from 0 to 4".
  • p. 2 Methods, Patients
score of <4 on the Full Outline of Unresponsiveness [FOUR] scale,12 which ranges from 0 to 4
  • p. 5 Table 1 footnote ‖
Full Outline of Unresponsiveness (FOUR) motor scores range from 0 to 4

Only the authors can say what is right here.

Judge's note: Rests on outside knowledge: the full FOUR scale has four components scored 0–4 each, so the total runs from 0 to 16. Page 2 Methods and page 5 Table 1 footnote ‖ were checked in the crops. In the Methods the clause 'higher scores indicating better motor function' shows the motor subscale was meant, but the wording says 'FOUR scale' while Table 1 says 'FOUR motor score'. This is an imprecise label, not a wrong criterion.

Wording Table 2 footnote * refers to "poor neurologic function at 6 months", but the row and Methods call this outcome "Poor functional outcome at 6 mo".
  • p. 9 Table 2, footnote *
and poor neurologic function at 6 months were adjusted
  • p. 9 Table 2, row label
Poor functional outcome at 6 mo‡
  • p. 9 Table 2 footnote *
poor neurologic function at 6 months

Judge's note: Page 9 image: footnote * lists 'death from any cause, a modified Rankin scale score of 4 to 6, and poor neurologic function at 6 months', which corresponds to the 'Poor functional outcome' row. The Methods (main.txt line 228) define 'poor functional outcome'. This is a terminology inconsistency only.
