Ventilation with Lower Tidal Volumes as Compared with Traditional Tidal Volumes for Acute Lung Injury and the Acute Respiratory Distress Syndrome

New England Journal of Medicine, 2000 · paper · Clinical trials

A multicenter randomized trial comparing lower (6 ml/kg) and traditional (12 ml/kg) tidal volumes in mechanically ventilated patients with acute lung injury or ARDS. Stopped at 861 patients, it found lower mortality (31.0% vs 39.8%) with lower volumes.

3 minor (typos and the like; most checked by the judge only)
Wording Text: "The respiratory rate was significantly higher in the group treated with lower tidal volumes on days 1 and 3" omits day 7. Table 3 shows day 7 respiratory rate 30±7 vs 20±7, and the footnote does not list it as non-significant.
  • p. 4 Results, paragraph on respiratory values
The respiratory rate was significantly higher in the group treated with lower tidal volumes on days 1 and 3
  • p. 5 Table 3, respiratory rate, day 7
    30±7

Judge's note: Checked page 4 text and Table 3 with its footnote on page 5. The footnote says all differences were significant (P<0.05) except mean airway pressure on days 1, 3, 7; PaO2:FiO2 on day 7; minute ventilation on days 1 and 3; pH on day 7; and PaO2 on days 1, 3, 7. Respiratory rate is not in that list, so by the table it was also significant on day 7. The text's restriction to days 1 and 3 seems to be framed around the minute-ventilation contrast. That makes it incomplete rather than false, so this is a minor consistency point.

Wording "The mean log-transformed plasma interleukin-6 values decreased from 2.5±0.7 pg per milliliter on day 0 to 2.3±0.7 pg per milliliter...": the log-scale values carry the linear unit, and the log base is not stated.
  • p. 5 Results, last paragraph (right column)
The mean log-transformed plasma interleukin-6 values decreased from 2.5±0.7 pg per milliliter

Judge's note: The crop on page 5 confirms the wording. The values are means of log-transformed concentrations, but they are labelled 'pg per milliliter'. The Methods (page 3) say only 'log-transformed', without a base. This is a common shorthand, but it leaves the scale ambiguous, so it is minor.

Typo Reference 7 reads "Gattinoni L, Presenti A, Torresin A, et al."; the co-author's surname appears to be Pesenti.
  • p. 8 References, ref. 7
Gattinoni L, Presenti A, Torresin A, et al.

Only the authors can say what is right here.

Judge's note: Rests on outside knowledge: the second author of the J Thorac Imaging 1986 paper is A. Pesenti. The page 8 image confirms 'Presenti' is printed, so this is not an extraction artefact.

Second check: none (judge only)
