Effects of Extended-Release Metoprolol Succinate in Patients Undergoing Non-Cardiac Surgery (POISE Trial)
2008 · paper · Clinical trials
A randomized trial of extended-release metoprolol versus placebo in 8351 patients having non-cardiac surgery. Metoprolol lowered the combined rate of cardiovascular death, heart attack and cardiac arrest, but more patients died or had a stroke.

Substance Results: "Of the 60 strokes that occurred in the metoprolol group, 49 were ischaemic and three were haemorrhagic". Table 3 gives 41 strokes for metoprolol and 19 for placebo; 60 is the total for both groups.

Of the 60 strokes that occurred in the metoprolol group,
Judge's note: Checked on the page images. Page 4, Table 3 stroke row: 41 (1·0%) metoprolol, 19 (0·5%) placebo. Page 7, Table 5: stroke 60 (0·7%) of 8351. The subtypes on page 5 sum to 60 (49+3+8). So the text gives the pooled total to the metoprolol group.

Reference or label Figure 2 y-axes are labelled "Risk (%)" but the ticks run 0–0·08, 0–0·020 and 0–0·03, which are proportions.
- p. 5 Figure 2, y-axis labels of panels A and C
Risk (%)
Judge's note: Checked on the page 5 image. In panel A the curves end near 0·06 and 0·07, which matches the primary-outcome rates of 5·8% and 6·9% in Table 3. In panel C metoprolol ends near 0·010, matching 1·0%. The tick values are proportions, so the '%' unit in the label does not match the scale.

- p. 3 Table 1, Undergoing major vascular surgery, metoprolol
1500 (36·0%)
Only the authors can say what is right here.
Judge's note: Checked on the page 3 image. 1500/4174 = 35·94%, which rounds to 35·9%. Nearby cells in the same column match N=4174 exactly (for example 1549 gives 37·1% and 260 gives 6·2%). A denominator of 4172 or less, i.e. a few patients with missing data, would give 36·0%, but no missing-data denominator is stated anywhere. Only the authors can say which applies.

Judge's note: Checked on the images. Table 1 (p3) has '>70', and the methods text has 'age >70 years'. Table 5 (p7) has '≥70'. I found no statement that Table 5 uses a different age cut-off, so the authors should say which threshold is correct.

Wording Discussion: "three from undergoing cardiac revascularisation" per 1000. The raw counts in Table 3 (11/4174 vs 27/4177) give 3·8 per 1000.
- p. 7 Discussion, left column, per-1000 estimates
three from undergoing cardiac revascularisation
- p. 4 Table 3, Cardiac revascularisation
11 (0·3%)
Judge's note: Recomputed from the Table 3 counts on the page 4 image: 3·8 per 1000, which rounds to 4. Every per-1000 figure in the paragraph, including 3, is reproduced by subtracting the rounded Table 3 percentages: 0·6-0·3, 5·7-4·2, 2·9-2·2, 3·1-2·3, 1·0-0·5, 15·0-9·7, 6·6-2·4. So 'three' follows from a consistent, if imprecise, method. Minor.

The paper gives no definition or analysis set that would explain 351. Webappendix 2, which describes the model, is not among the files.
