Supplemental Perioperative Oxygen to Reduce the Incidence of Surgical-Wound Infection

2000 · paper · Clinical trials

A randomized trial of 500 colorectal surgery patients given 30 or 80 percent inspired oxygen during and two hours after surgery. Wound infection occurred in 5.2 percent with 80 percent oxygen versus 11.2 percent with 30 percent.

Reference or label Introduction cites the range "the use or nonuse of prophylactic antibiotics,5-7", which takes in ref 6 (Van Oss et al., "Effect of temperature on the chemotaxis, phagocytic engulfment, digestion and O2 consumption of human polymorphonuclear leukocytes"), a paper about temperature, not antibiotics.

Judge's note: Page 1 shows superscript 5-7 after 'prophylactic antibiotics' and only 2 after 'patient's temperature during surgery'. In the page 7 reference list, refs 5 (Burke) and 7 (Classen) are about antibiotic prophylaxis. Ref 6 (Van Oss) is about the effect of temperature on leukocyte function, and I found no other place in the text that cites it. It looks as if ref 6 belongs with the temperature clause. Only the authors can confirm the intended placement.

4 minor (typos and the like; most checked by the judge only)
Wording "Scores were determined before surgery and randomization", but SENIC as described includes "surgery that lasts two or more hours" and a contaminated or infected wound, and NNISS includes "the duration of surgery", which cannot be known before surgery.
  • p. 2 Evaluation, risk scales paragraph
Scores were determined before surgery and randomization.
  • p. 2 Evaluation, SENIC definition
surgery that lasts two or more hours

Judge's note: Page 2, Evaluation section. Both scales as defined in the paper include components that are only known at or after the end of surgery (duration; wound contamination for SENIC). Table 1 reports full SENIC and NNISS scores under 'Postoperative'. The paper does not say whether predicted durations were used. This is an unresolved ambiguity for the authors to clarify.

Wording Table 1, intraoperative heart rate "78±13" vs "75±11" (N=250 each) with "0.35": the printed summary statistics and P value do not correspond under the declared test (unpaired t-test or Mann–Whitney).
  • p. 4 Table 1, Intraoperative, Heart rate row
    78±13
  • p. 4 Table 1, Intraoperative, Heart rate row
    0.35
  • p. 4 Table 1, Intraoperative, Heart rate
    Heart rate — beats/min 78±13 75±11 0.35

Only the authors can say what is right here.

Judge's note: From the page 4 image, Welch t^2 = 9/(169/250+121/250) = 7.76, so t ≈ 2.79 and P ≈ 0.005. Even with the means rounded so the difference is only 2, t ≈ 1.86 and P ≈ 0.06. The Methods (Statistical Analysis) declare t-tests or Mann–Whitney U tests, and for near-normal data a Mann–Whitney test would not reach 0.35. Other rows I checked reproduce within rounding: weight 72±17 vs 74±17 gives P≈0.19 against 0.26 printed; MAP 88±11 vs 89±10 gives P≈0.29 against 0.25. This is a query for the authors.

Typo Awkward wording: "had their staples removed one day later after surgery than patients with no infection".
  • p. 5 Results, paragraph on Table 3
had their staples removed one day later after surgery than patients with no infec-

Judge's note: The page 5 image confirms the wording. The meaning is clear (staples were removed one day later), so this is cosmetic.

Judge's note: Rests on outside knowledge: the first author of the Adv Exp Med Biol 1984;169:447-55 Po2-histogram paper is W. Fleckenstein. The page 7 image confirms that 'Fleckstein' is what is printed.
