Effect of Early Tranexamic Acid Administration on Mortality, Hysterectomy, and Other Morbidities in Women with Post-Partum Haemorrhage (WOMAN)

2017 · paper · Clinical trials

An international placebo-controlled trial of tranexamic acid in about 20,000 women with post-partum haemorrhage. Death from bleeding was modestly reduced, especially with treatment within three hours of birth; hysterectomy and the combined death-or-hysterectomy outcome were not reduced.

Substance Hysterectomy result printed as "358 [3·6%] ... vs 351 [3·5%] ... RR 1·02, 95% CI 0·88–1·07; p=0·84" (Abstract p1 and Results p6). The upper limit 1·07 does not fit the counts, which give about 0·88–1·17.
  • p. 1 Abstract, Findings
    RR 1·02, 95% CI 0·88–1·07; p=0·84
  • p. 6 Results, hysterectomy paragraph
    RR 1·02, 95% CI 0·88–1·07; p=0·84

Only the authors can say what is right here.

Judge's note: I checked both page images (p1 Abstract Findings, p6 Results right column), and the interval is printed identically in both places. From 358/10036 vs 351/9985 I get RR=1·015, SE(log RR)=sqrt(1/358-1/10036+1/351-1/9985)=0·0738, and a 95% CI of exp(0·0147±0·1446)=0·878–1·173. The printed lower bound and p=0·84 (z≈0·20) fit this calculation; the printed upper bound does not. On the log scale the printed interval runs about −0·15 and +0·05 around the estimate, so it cannot be a Wald-type interval around 1·02. No adjusted hysterectomy analysis is reported that could account for it, and there is no erratum in the files. 1·17 is the likely intended value, but only the authors can confirm it.

Substance Results p6 prints "546 [5·6%] placebo group" for the composite primary endpoint; the Abstract and Table 3 print "546 [5·5%]" / "546/9985 (5·5%)".
  • p. 6 Results, primary endpoint paragraph
    546 [5·6%] placebo group
  • p. 1 Abstract
    546 [5·5%]
  • p. 6 Table 3, All patients
    546/9985 (5·5%)

Judge's note: Seen on the p6 image (both the text and Table 3) and in the p1 abstract. The count, population and endpoint are the same in all three places. 546/9985 = 5·47%, so 5·5% is correct and the 5·6% in the text is not.

Report a problem with this finding
Substance Results p5 say "20 002 (99·7%) received the first dose ... (10 037 received tranexamic acid and 9975 received placebo; figure 1)", but Figure 1 shows "10 027 received allocated dose 1", and 10 027+9975=20 002.
  • p. 5 Results, first paragraph
10 037 received tranexamic acid and 9975 received placebo
  • p. 4 Figure 1 trial profile
    10 027 received allocated dose 1

Judge's note: Checked on the images of p5 and p4. The text's own total, 20 002, requires 10 027. The 10 037 in the text conflicts with both that total and Figure 1, which the sentence cites.

Substance Results p5: "20 002 (99·7%) received the first dose ... (10 037 received tranexamic acid and 9975 received placebo; figure 1)". 10 037 + 9975 = 20 012, and Figure 1 gives "10 027 received allocated dose 1".
  • p. 5
of whom 20 002 (99·7%) received the first dose of the

Judge's note: On p5 the text prints 10 037. Figure 1 on p4 prints 10 027 for the tranexamic acid arm and 9975 for placebo. 10 027 + 9975 = 20 002, which matches the printed total, so the 10 037 in the text looks like a transcription slip for 10 027.

Judge's note: Reference list on p12 checked: 17 is the WOMAN SAP and 18 is the composite-outcomes BMJ paper. Ref 17 is first cited only in the Discussion ("post-partum haemorrhage death.17"), after ref 18 has already been cited twice in the Methods, so the citation order is broken. Ref 18 is cited appropriately in the Discussion on all-cause mortality. The Methods sentences describe the published SAP and its prespecification, which is ref 17.

Wording The primary outcome is defined as "hysterectomy within 42 days of randomisation" in Methods (p3), but as "within 42 days of giving birth" in the Abstract, Results (p6) and Discussion.

hysterectomy within 42 days of giving birth occurred in 1080 women
  • p. 1 Abstract Methods

Only the authors can say what is right here.

Judge's note: Methods Outcomes (text lines 263–264) says randomisation, and follow-up is also stated as 42 days after randomisation. The Abstract, the Results p6 and the Discussion say giving birth. Some women were randomised more than 3 h after birth, so the time origin is stated two ways.

Only the authors can say what is right here.

Judge's note: The Figure 5 image on p9 shows 'p<0·0000*' beside the >3 hours Overall row. A p value cannot be below 0, so the intended threshold and test (presumably the heterogeneity between time strata) need the authors to clarify.

Only the authors can say what is right here.

Judge's note: The p12 reference list image confirms that the R1 version and 1996 date sit with a URL to the R2 document, so the entry is internally inconsistent about which version is cited. The 'International Conference' (p2 text) versus 'International Council' (reference) difference is not counted as a defect: the text names the body that issued the GCP guideline, and the reference uses its current name. The claim that the organisation was renamed rests on outside knowledge.

Wording Placebo composite primary endpoint is "546 [5·5%]" in the Abstract and Table 3 (546/9985 (5·5%)) but "546 [5·6%] placebo group" in the Results text on p6. 546/9985 = 5·47%, so the Results-text figure is the one that is off.

Judge's note: Abstract p1, Table 3 p6 and Results text p6 all checked on the images. The abstract value the cluster quotes is correct. The inconsistent value is in the Results paragraph on p6 (text line 707), where 5·6% is printed for the same quantity.

Only the authors can say what is right here.

Judge's note: Seen on the p4 Figure 1 and on Table 3 (p6). Table 4 and Figure 4 also use 10 032, while the death analyses (Table 2, Figure 3) use 10 036. The methods (p4) mention a complete-case analysis with no imputation, which could account for 4 women missing hysterectomy or surgery data. Judge's note: Seen on the p1, p6 and p9 images. A crude log-RR interval from 89/7520 vs 127/7408 gives SE about 0·137 and an interval of about 0·527–0·903, which matches Figure 5. The text's 0·52–0·91 may come from a different method (for example a regression model), but neither place says so. Query: which method or model gives each interval?

Only the authors can say what is right here.

Judge's note: Seen on the zoomed Figure 2B crop. The hour-0 bar totals about 248 and its bleeding segment tops out near 191. For deaths, the text's '43 within 1 h' matches the full hour-0 bar in Figure 2A (about 40 + 3). Under the same convention, 191 matches only the bleeding hysterectomies. The figure is also hard to reconcile internally: its footnote excludes 311 pre-randomisation hysterectomies, yet the bars sum to about 600. Values read from a bar chart are approximate, so only the authors can say which number is meant.
