Cardiovascular and Renal Outcomes with Empagliflozin in Heart Failure

Heart & Lung, 2021 · paper · Clinical trials

A double-blind trial of empagliflozin versus placebo in 3730 patients with heart failure and reduced ejection fraction. Cardiovascular death or heart failure hospitalization occurred in 19.4% versus 24.7%, regardless of diabetes, and kidney function declined more slowly with empagliflozin.

Reference or label Discussion: "the effects of dapagliflozin in the DAPA-HF trial.5" cites reference 5, which is the EMPEROR-Reduced rationale and design paper (Eur J Heart Fail 2019;21:1270-8); the DAPA-HF report is reference 4 (McMurray et al., "Dapagliflozin in patients with..."), as cited in the Introduction.
  • p. 9 Discussion, second paragraph
    DAPA-HF trial.5 The current trial was enriched
  • p. 2 Introduction, second paragraph
    gardless of the presence or absence of diabetes4;
  • p. 12 Reference list, entries 4 and 5
    rationale for and design of the EMPEROR-Reduced trial. Eur J Heart Fail 2019;21:1270-8.
  • p. 9 Discussion, paragraph 2, first sentence
    DAPA-HF trial.5

Judge's note: Viewed the page 9 crop: the superscript 5 after "DAPA-HF trial." is clear. The page 2 crop shows the Introduction cites DAPA-HF with superscript 4. The reference-list crop shows entry 4 is McMurray, Solomon, Inzucchi et al., Dapagliflozin in patients with..., and entry 5 ends "rationale for and design of the EMPEROR-Reduced trial. Eur J Heart Fail 2019;21:1270-8." The pointer should be 4.

1 minor (typos and the like; most checked by the judge only)
Typo Results text reads "the estimated GFR declined by –0.93 ml per minute per 1.73 m2 (95% CI, –1.97 to 0.11) in the empagliflozin group and by –4.21 ml per minute per 1.73 m2"; "declined by" a negative value is a double negative (a change of –0.93 and –4.21 is meant).
  • p. 7 Results, Other Prespecified Outcomes
    the estimated GFR declined by –0.93 ml

Judge's note: Viewed the page 7 image: the minus signs before 0.93 and 4.21 are printed, following "declined by". The intended meaning is clear from the confidence intervals, so this is a wording blemish only; either "changed by –0.93" or "declined by 0.93" would be consistent.
